Transcript requests in the 15th Judicial District are governed under Administrative Order 24-5. Below are the standardized forms for the District.

Court appointed counsel for felony indigent cases should use BIDS forms, located at: https://www.sbids.org/forms
Self Represented Litigants may file a Poverty Affidavit with their transcript request and proposed order. Poverty Affidavit, EnglishPoverty Affidavit, Spanish All transcript requests are to be completed in writing. Please file requests with the Clerk of the District Court in the county in which the preceeding was held to request a transcript.
